Forms unavailable: Patients need permission to fill out the form now

    Good Afternoon,

    All of a sudden, our patients aren't able to fill out our forms online. They are being told they need permission. How do we fix this?

    Thank you for contacting us.

    Is this happening on all of the forms or just specific forms?
    If you open the form while you are logged out, are you able to view the forms yourself?

    I can confirm that your forms are set to public, and they should be viewable to everyone.
    I was able to view the forms without logging in:

    Are you embedding the forms on your website? If so, could you please give us the address so that we can investigate further.
