Updating form submissions and creating QR codes

I would like your help in creating a form that allows the patient to submit a registration form, but that form can then be updated by the health care worker. We need to add results to a patient's form. Can we also have a QR code associated with each profile, so that their results from us pop up

As for creating a QR code that will be associated with every submission and having that QR code included on the submission itself, I'm afraid that it would not be possible. But are you looking to include the QR code in the email being sent to you for every form submission? Kindly confirm so we may start running some tests to see if we can find any workaround that will meet your requirements. We will wait for your response.
