    Looking for Multiple payment order form template

    I am looking for an order form template I saw earlier which shows payment types for cash, check, or paypal, where each option is hidden until checked. I know there is an existing template, but I'm unable to find it. Can you point me to the right form? I'm trying to create my own, but it's been a real pain.

    Thanks. I was looking at that guide. I have other questions:

    Is there a way set up product options from the Check Box field? For example, with the paypal, I can set up Product 1 and then size and color for Product 1. I can't seem to do that with a Check Box field (as I don't want to use the paypal option for that field).

    Can I set up a Check Box for cash or check payments and get those fields to total?
