Why is my form not displaying on my form?

    Good Afternoon,

    (This is my second attempt to send this, as the first resulted in a modal window that said 'Slow down, you're moving too fast)

    I was informed by one of our customers that our Contact Us form was no longer active. Upon logging into the back-end of our website and our Jotform account, it seems as if the account has been suspended/disabled by your 'phishing prevention system'.

    I attempted to take a look at the form myself to see if there was anything I may have missed or could have triggered it, but upon logging in am immediately met with a 'your account has been suspended' screen and am unable to progress any further than your Contact section. I'm not sure what could have caused this, as the information we request from those filling out our Contact Form is rather straightforward and common for a B2B manufacturer/distributor: First/Last name, Company they work for, email address, phone number, and city/state. Location information is used to assign the contact to the appropriate representative over their geographical location.

    We do not ask for any sensitive information such as social security number, credit card info, or any form of log-in.

    I appreciate any help/information you could provide to get this resolved!

    I see that the account everkemproducts is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I have checked your forms and they are clean and normal. I have activated your account and you should be able to access your forms normally. We are sorry for the trouble caused.
