Thank you for reaching support,
It is because the Access Settings on the form is set up to only allow users with domains that contain: info@isofficia.org
So unless your email is "test@info@isofficia.org" you won't be able to access it.
You may change it to Public if you want to and just set up Password protect on the form for privacy: https://www.jotform.com/help/579-how-to-password-protect-a-form
