For your information, I have cloned your form to demonstrate the answer to your question and nothing has been changed on your original form. You can edit the "Autoresponder" e-mail section and add the time and date booked by following the steps below. Please see the screencast under the steps:

    1. Go to the "My Forms" page and click "Edit Form" from the right of the form.
    2. Click the "SETTINGS" tab on the top of the page of your form. Then select the "EMAIL" menu.
    3. Hold your mouse on the "Autoresponder" tab, under the "EMAIL" menu, and click the green pen icon to edit the Autoresponder e-mail.
    4. In the "Email Content" window, click the table and select the "Insert row before/after" option under the field you want the Appointment details to be shown. Name the row by clicking the left cell of the row. And then click the right cell of the row you inserted, click the "Form Fields" button on the top right of the "Email Content" window. Select the "Appointment" field. After that, the Appointment details will be shown in the Autoresponder mail the user gets.
