Error in submission process

    I have literally spent 4 plus hours filling out intake forms for a doctor and each time I try to submit the form I get an incomplete error message. Unsure of why because all those sections have been completed...items have been rated/ranked. How can we rectify this?

    Also, is there a way to avoid losing all this valuable information that I've spent hours typing out? I've just kept the window open for now and the computer on sleep.

You can get in touch with the owner of the form. Or we can help if the form owner reaches us, and provide the Form ID of the form you had the issue with.
