Unfortunately, we have no control over the data being returned to the QR code from an external source. Can you describe the workflow of your form? Perhaps we can suggest a workaround if we understand the way it works.

Hi there,
To my understanding, you wanted to scan a QR code, and after the widget reads the code, it will auto-populate the name, address, city, state, and zip code associated with that QR code.
I'm afraid it's not possible with the QR code widget as the widget only reads data from the QR code. It does not support trimming of data and populating them into different fields.
You can use the QR code widget and pair it up with the SpreadSheet to Form widget. Once the QR code widget scans for a unique code, you can copy the scanned code to the Spreadsheet to Form widget. The Spreadsheet to Form widget will then populate the fields from the data on your spreadsheet file tied up with the QR code. Here's a sample output:

You can check my demo form to check it for yourself: https://form.jotform.com/203493984839069.
Kindly use this QR code for the widget to scan:
Please confirm if this workaround works for you so I can guide you through the steps on how to build the form.
We look forward to your response.
