    Good morning,

Our practice is interested in integrating JotForms onto our website to give our patients an easier time filling out paperwork.

    I had a few questions:

Do we need to upgrade to the Silver/gold package to become HIPPA compliant?

If we do not upgrade to the Silver/Gold package are any forms we use not considered HIPPA compliant?

Is the current form we created going to be usable if we upgrade to this package or do we need to take additional steps?

If there are additional steps we must take after upgrading to the HIPPA compliant package, what are they? are forms not HIPPA secure by default when using this package?

Do we need to upgrade to the Silver/gold package to become HIPPA compliant?

Yes, you need to upgrade your account to a Silver or Gold plan to use HIPAA Compliance.

If we do not upgrade to the Silver/Gold package are any forms we use not considered HIPPA compliant?

If you do not upgrade to the Silver or Gold plan and enable HIPAA Compliance, your forms will not be HIPAA Compliant.

Is the current form we created going to be usable if we upgrade to this package or do we need to take additional steps?

If you upgrade to HIPAA Compliance, your existing form will be moved to our HIPAA servers and they will be usable.

If there are additional steps we must take after upgrading to the HIPPA compliant package, what are they?

After you upgrade your account to the Silver or Gold plan, you need to enable/ upgrade to HIPAA Compliance.

Are forms not HIPPA secure by default when using this package?

Your account is currently Free and forms on the Free accounts are not HIPAA Compliant by default, but they are secure. If you are referring to the Silver or Gold plan, you need to enable/ upgrade to HIPAA Compliance after the upgrade.

    Thanks Nik!

    A few more quick question:

If we integrate our current web form with our website as a test and then decide to update to the Silver/Gold plan and opt into the HIPPA secure option, will it change anything as far as the user experience goes externally (Those filling the form out) and internally (those going through the form data send by patients)

Our website we plan on using JotForms with is a WordPress site. Once this form is integrated into the site, how do our team members actually receive the data patients fill out? is there a portal we need to use or can we set up some type of email forwarding? What is the normal method for doing this?

If we upgrade our plan, are there any SLAs? Or do we get a different support package?

Once we upgrade to the gold or silver plan, then opt to change to the HIPPA secure plan, are there any additional costs associated with that?
