Totally disaster on my form

Hello, i launch a landing page with jotform. I checked something like 20 times if everything was okei and then Iaunched.
After 5 days i discover thanks to a visitor that upload form was not working properly!!
I lost budget, time and credibility with my visitors!

The people wrote to me that doesn't work when they pressed on upload bottom on upload photo widget. Now the form does working because I duplicated the upload section and deleted the old one. Now the point is that also the old one before was working until the bug, how can i be sure that will not be another bug?
