Creating forms for pharmacy COVID Vaccine

    We are an independent pharmacy. We expect to be receiving COVID vaccine in the next 1-2 weeks. We are wanting to streamline the process as much as possible before the vaccine arrives to be prepared. There are 2 things I'm needing to accomplish online before the patient gets to the pharmacy.

    1. Fill out & submit a form like the one attached.
    2. Schedule an appointment online for administration of the vaccine.

    Please advise me on where to start and if you guys can help with this!

    You may click on the 'Create Form' button at left-top corner to start creating.

    In addition to the one above, I assume you have a PDF File for the screenshot you have shown.

    If yes, you can import it so the system can create an online form for it. You can then add, remove, and map online fields to your PDF file. Please see this guide to help you get started:

    Then, within the same form, you can add an Appointment field so users can select a time slot.
