Performance review template

This article about performance reviews made me wonder if you have something similar - probably simpler - that could be a template for graduates to evaluate a training as part of the end of their experience. I'm an adult trainer and I'm looking for a quick, easy, engaging way for them to evaluate the training material and me as their facilitator. I used to use QuestionPro but it's too expensive since I may only teach once a year.

Thank you for the link to your performance reviews templates. As a teacher of adults, after looking through them all, I can see that I can do what I’d like to do with Jotform. The Trainmore Feedback Form was the only one that came close to what I’d like to create but the others showed me the scope of the type of questions that are available.
Before going further, it would be helpful for me to know the following.
Is it possible for me to customize that template or does JotForm have to do that? I’d like something I can customize myself.
Is there a charge to use such a JotForm? If so – is it dependent on numbers of students responding over time, or how is the fee worked out?
If I use a JotForm in this way, can I just add a link to an email to graduates or does it have to be placed in my website? For greater ease of use, I’d prefer to be able to add the link to an email when required.
What else should I know about how this all works that I don’t know to ask?
