Pass date, time, AM/PM to another form using URL parameter

I'm trying to pass the date, time and AM/PM information from one field to another. The following string will pass the date, but not the time and AM/PM:

I saw in another post that I have to remove the underscores (_) and hashtags (#) in order to receive data, hence why you don't see any in the script above. Any help would be greatly appreciated.

Upon checking your form, it seems like you are using the wrong URL parameter for the time and am/pm.
