update username

    Can we be NurseDash instead of Anum, she is no longer with company.

    We can update your username however, the username NurseDash has been taken already.

    Kindly provide at least 3 unique username so that we can check for its availability.
    We will use the first available username to update your account.
    Please take note that period "." and spaces are not allowed anymore.

    Note: If you are using custom URL (how to create a custom URL of a form) for your form, it will no longer work since the username got changed. You will need to change the URL to have the new username in it instead.
