Good morning! Let me start off by saying that Jotform is a God send! We absolutely love it.

Unfortunately, I'm having a bit of trouble with the last couple pages of my form and am looking for some assistance. I've attached a copy of the way the forms are printing. I did cross out names/phone numbers for confidentiality purposes.

Do you have any recommendations on how to fix this issue? These to forms are extremely important to legal opporating in the world of childcare and we would really like to continue with out "one stop shop" form enrollment forms.

    Thank you for your help!

Hi activemindsacademy, thank you for reaching us.

I have cloned your form, did a test submission and was able to recreate the issue. I have also tried to submit the form without any filled fields but it seems like the last 2 pages are still showing blank.

However, I managed to fix it by updating the PDF.

Could you kindly try to update your PDF? You may do so through Upload > Update PDF > Update this PDF file.

You may also want to try to do it in cloned form just in case. To clone a form, you may refer to the following guide: How to Clone an Existing Form from a URL.

Please give it a try and let us know how it goes.

    Hi Jessica,

I had updated the PDF File and had completed it as a test, it does appear that it is still completing like the original form did.

    Thank you!

Hello! This is the cloned form of my colleague's form:

She was able to fix this issue. You can clone this form into your account and start using it instead of your original form.
