How do I place a signature option on to the PDF file?
It did automatically for some files but not others. How do I manually create that option?

You can add a Signature field to your web form then position it to the original PDF where you want it to appear. So when you sign the form and preview the PDF, you'll see the signature image in the PDF.
Example:
