How do I make a form parents can initial and pay?

I need help with creating a contract that parents can sign and initial online. We will also like to put a pay button so they can pay as well.

You can put a basic field to ask for their names, email etc.. and put a signature field to have them sign. Then you can add a payment field and integrate your payment account with your Jotform.
