My forms are disabled.

    I enabled the form status but when I test the link, it still doesn't work.

    I did check your account and found that it is currently suspended as your form "Client Intake Form" is suspected to be used for phishing. You seem to have added password fields in form. Please check the screenshot below:


    Authentication form can't be used in Jotform and creating such forms are against the Terms of Use. The password fields from your form needs to be deleted. I have deleted the guilty fields from your form and activated your account again.

    Kindly refrain from creating form with email/password, login/password or credit card details.
