    How can I add textboxes next to the checkboxes option text?

    How can I make the section of the attached form, that says: 'My current rate for ___ coverage is:'  have a filllable field where the blank lines are (next to the words: single, husband and wife, single parent and child, full family, other) that our clients can type in the dollar amount of their amount of coverage?

    Is this what you want to accomplish?

    If so, let me inform you that cannot be done from our form builder, however it can be accomplished by getting the Full source Code of your form and adding some HTML code. Please use this guide to know how to get the source code of the form: How to get your form source code.

    However, if you prefer I can proceed to make that change to your form in that way you will save all that work. It will be a pleasure to do this work for you. I'll get the source code of your form, will make the changes and will get back to you with the right code once I get it ready.

    The only thing you need to do is to create an HTML document and paste the code of the link between the tags <body> and </body>

    I do need help.  I am new to all of this, so I dont' know how to create an HTML document.

    If you are going to embed your form on your website, you can use the modified script with the changes made by Eliezer: http://pastie.org/pastes/8392953/text

    Or, if you want to use your form URL, you have to copy that source code, paste it to notepad and save it as .html file e.g. myform.html then upload it to your server.
