Wrong information printing on downloaded form

I created a form using the clone function. The name, header, and several other items were changed. I published the form and received a response. When I went to print the form the header shows the original version of the form with different header information. This is the first time this has occurred since I have done this in the past. Any ideas?

I cloned and tested the form and I was able to receive the right formatting for the header:

Can you please delete the previous PDF document and create a new one?
