Add images choices to the form?

HI there,

We are hoping to be able to add this form to our website, but we'd like to be able to add images....not necessarily as a product,

For example:

Customer wants to mail us a Quilt Top, we need to know what color thread they want, what quilting design they want.

We want to say, here is your options:

Hearts and Swirls

Nautical

Feathers

Can it then have another drop down once they've selected the "main category" and then they have 5 choices from there?
