Unable to Log-in due to Suspension

    My username is Brooks_Debbie . I am not able to log in. When I tried password reset, I did not receive the email.

    How can I log in?

    Hi,

    Your account is currently suspended. This form (https://www.jotform.com/form/203193629948063) triggered the suspension.

    Collecting credit card details using regular text fields is not allowed here in JotForm. Please check our terms of use.

    If you allow us to delete the related fields, we can activate your account.

    If you need to collect payments from your users, please use one of our payment processors. Here's a guide: Setting Up Your First Order Form

    Yes, please delete those fields. I did not read the terms carefully.
