Why are entries missing required fields?

    Hi there.

    I am unsure if this is an issue due to us using the free trial and not having an established account, but we are a medical clinic with an IUD/Women's clinic developing upstairs.

    We use Jotform for questionnaires to find out medical history.

    I have created the perfect form for what we need, and when I edit it, it is fine. I re publish it every time and turned ON the setting that makes every questions mandatory without skipping and submitting, but for whatever reason our forms are still missing questions.

    Patients are submitting them and my Dr. is complaining that it is always the same two questions that are just gone.

    Is there a way to fix this?

    These required fields should work on whatever account. What are the required fields sent without filling? Is the "What kinds of birth control have you used in the past? (Including condoms) Have you experienced any side effects?" one of them, right?


    I want to recommend two different things:

    1. Access the conditions of your form here and clean the errors by completing the missing fields or deleting the unnecessary conditions;
    2. Change your form theme to the new Default Layout;


    After making these changes, please, monitor the new submissions. If the issue stills consistent, get back to us to investigate it further.
