I want to get a sample of the PDF document of the submissions

My hospital is currently using the system to build forms on the website, and i would like to get an account for my department also.  I was hoping to get a sample of what the information would look like if returned as a PDF after it is completed.  Please let me know how i can get this.

Please look at our PDF Template Gallery: https://www.jotform.com/pdf-templates/. The PDF copy of the submissions can be formatted similar to how these PDFs are created.

First, create the form with all the fields in it. Once created, customize the PDF document copy through our PDF editor. Here are our guides for PDF document:

The PDF document can be attached to the notification email that you will be receiving for every submission.
