How to create a form from a "Home Features Database"

We are a Custom Home Builder that has an archive of hundreds of one-of-a-kind plans we have previously constructed. We use a Questionnaire with new clients to discuss which features they want in their new home. We want to link this questionnaire with our Home Features Database to identify which of our existing Floor Plans best matches the desired features from the Questionnaire.
