1- How do I disable Lite Mode:

I see you are referring to the Date Picket fields. Since you have enabled Accessibility, as instructed, kindly disable Lite Mode within the Date Picker field:

You may simply use the Text element on the "Quick Elements" tab of the add form section: 

This allows having text without an input.

Please note that the Accessibility feature depicts all kinds of disability and that signing may very well be difficult for certain users with certain disabilities.

The form will work and can be signed, however, it is not possible to remove that warning since you have enabled the feature and it is a contradiction to the feature.
