I've tried embedding each of the codes in my Microsoft Word docx where I insert a hyperlink, and I get an error message "Cannot open the specified file". What am I doing wrong?

You cannot embed the form itself on an MS Word Document -- the most you can do is add the link URL to the form, like this http://www.jotform.com/32826112558959 (click on the link)
To be able to embed a form on a web page, it requires the existence of html placeholder or widget at the least.
