My jotform account has been suspended for using copyrighted logo

    My jotform account was suspended bec there is an item that is unacceptable and appears to be phishing. Can you pls tell me what field to delete to reactivate my account?

    Hello Bernice! Your account is suspended because you're using a copyrighted logo on your form (screenshot provided below for reference).


    Your form could be violating the copyright law by using the image logo. Can you give us more information that you are associated with this company/store?

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately. 

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    3. Share with us a document that will prove that you're authorized to use the said logos.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to with Thread #2785552 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can just remove the logos and lift the suspension of your account.
