Hi there!
I was wondering if it's possible to make the two question given below formatted to show this: For the first one "Standard Hourly Rate for Services Not Described Above ($):" to have the number as a currency or display the number like this "$6.89". And for the second question, "MyPM Customer Discount for Services Not Described Above (%):" to have the number as a percentage or display it like this "6.89%". I would appreciate any help in this matter.

Thanks for the help it worked! I did have one follow-up question.
Is there any possibility of formatting the box to look like the rest of the form?What you provided shows it in a white box, but I'm looking to maintain the consistency of my form. Any way in making the answer box look like the image below?Thanks! Happy New Year!
