Why are phone numbers not transferring over?
They are able to be entered in as you are completing the form
They are selected to transfer over but nothing
HIPPA security feature not on to block these
view attachments.

I can see there were changes made to the said fields. Could you please try deleting the existing a new email notification template and create a new one?
This will generate a new email notification with all the updated fields added to it.
Please try that and let us know if that'll work for you.
