Consent forms and intake

Hello,

I am interested in your silver and gold plans. I am using formstack now and wanting to switch. When I ask for a signature on my consent forms do I only see the signature or will the consent form be added to the entire signature line. With formstack I get the signature but not the consent part that the signature is attached to for my records. I am looking into becoming completely paperless and ease for my pts with ipad and phone.
