PDF file name on HIPAA forms

Hi,
After I updated my account to HIPAA compliant, I am no longer receiving the submitted PDF with the {Date} {Name}{Form Title} format which I wish for. I am only receiving {form title} in the PDF file name.
I understand the HIPAA security that is built into this. Even after toggling the aforementioned form fields PHI to off, the desired PDF file name is not achieved.
Please assist in creating the PDF file name with the {Date} {Name}{Form Title} format for forms:
COVID-19 Test Agreement & Informed Consent
and
Accureference Lab Requisition

Have you already changed the PDF filename in your Layout Settings in the PDF Editor?
If so, then delete the current pdf document and create a new one and customize it as shown above.

Yes, even after editing the PDF File Name field, the downloaded PDF file does not produce the desired PDF File name. See example below

This issue is not only on 1 form, however multiple forms.
I have attempted created new smart pdf forms from scratch without success.

I cannot clone your form. It's most likely that your form has been set to 'Private'.
Please set it to 'Public' in order for us to look deeper into the problem.

I changed settings to "allow others to clone". Both forms are set to public as well now. Thank you for your assistance

The PDF's file name is limited to form name and submission ID on HIPAA compliant accounts I'm afraid, even with disabled PHI.
