It could be a caching issue related. Please try to logout from your account and clear your browser's cache and log in again. After that try creating a new form and see if this problem is still there.
I also cleared the form's caches from end. Please let us know if this problem has not been solved with the above suggestion so we can further investigate on it. Thanks
