find form and question about discount

I have some questions about my account. I have a form which I send out to clients as an initial questionnaire but I can't find it on my account anywhere. Here is the link:

I wish to amend the form but I obviously need the original form to amend it.

Also, I operate 2 companies and am thinking of opening another account with you - will I still qualify for the 50% discount?
