create options to pay bill payments

Hi there,
there are three options.
pay 10 and bill insurance
pay 70 (no insurance)
pay other and you choose the amount.
You can not achieve this using a Product List. However, using normal fields like a single choice field or multiple choice field you can achieve this. Please check the guides below to see how you can assign calculation values to these fields and then pass the value to a payment field.

I would also love to hold a credit card number if they choose to bill insurance in case insurance won't pay.
As per our Terms collecting any details related to credit cards in normal fields is not allowed. You can use payment integrations to charge users instantly or later but still, you won't be able to see complete credit card details due to our PCI compliance.

One more question, how can i Assign sub users who can view the form
information but not make any edits or alternatively lock the form to avoid
people making changes.
Also is there a way to view daily results on the opening screen where I
can log in and see how many people are signing up each day?

It is possible to share your table submission and let your invited use to view the data only.

should ask you bill insurance or pay $70 and add the 70 to the total amount
at the end. can you do that?
