It is showing up for the admin, but not for the fans that have liked the page? I don't know why?!

Hi,

Did you checked if you have the form embed code embedded on the correct pages? Static HTML has two available option, the content and fangate. Kindly review.

You should also try to logged it to a different account and check the FB page there. If you could provide your FB page, we'd be glad to check it here.
