    Error message pops up on website form after hitting submit

    When I try to submit my form on this page, http://www.mymibs.com/contact-us.html I get the following error message - "

    Sorry! Only one entry is allowed.
    Multiple submissions are disabled for this form.

    I made 2 test submissions to your form but I couldn't recreate this behaviour. This Kind of messages are related with the form limits options, once the user configure a submission limit to the form, depending on the method used to control the unique submissions, you will get a message like that if you try to submit info more than once.

    I cannot see a form limit method configured in your form, I guess you already changed it, I'm right?

    Yes I fixed it but do you notice how on the form after it's submitted I the email there are a whole bunch of extra fields for some reason?
