Shows submissions, but are not their when I check for them.
Are you able to restore this form from the backup.
Is there a way we can back up forms ourselves in case of failure?

So sorry to hear about this issue. Various versions of a form can be can backed up by cloning the form. After cloning, remove the words "Clone of" from the beginning of its title and append the said title with the date and time of the clone's creation for easy reference. Submissions from the form being backed up are not saved with the clone. However, they are automatically backed up to our servers. You can also have them automatically backed up to your own computer by the method illustrated by my colleague Neil in this thread.
As to your more immediate concern of restoring the form in question, I will submit a ticket on your situation and you should hopefully get the form back within a couple days time.

Thanks, I tried restoring it by cloning but it doesn't work. So I still have a missing form.

No, I was referring to backing up forms in general, so that if something like this should happen to them, you would have backups to restore them from. Since the form in question has already vanished, it can't be cloned.
As to the other part of your query, there is not a ticket form to refer to but I have already submitted a ticket.

Wayne... my form just disappearded... What happened to it? I logged into the Jot site and it is as if there was never any form at all. How can I get this back?

I'll submit a ticket on this for you but I have a bad feeling it may be gone for good. Hopefully, I'm wrong.

Unfortunately, abajan's feelings is correct. We had a problem on our servers the day you created this form. So, the questions is lost. You will need to re-create this form. Sorry.
