    how can I set it up so a PDF saves that looks "JUST" like the screen with the radio buttons marked -and have a link or copy of that PDF

    I want the participant filling out our appliation to get a PDF of their form - as the administrator i need a copy of that PDF with the exact same look - radio buttons check or if the applicate didnt check - i want a copy or link to the form so on the admin side i can foward that form to peopl to review.  Last week you sent me instructions to add the header as such and i did that how ever it does not show the radio buttons - i want a carbon copy of the form - what it looks like on the screen when they hit submit.  

    Can you please help me figure out how to do that.
