I've check with my ISP and none of the suggested IPs are being blocked. Please advise if there is anything else that can be checked/tested.

As i have responded to your last thread post regarding this problem. Please try to use either of the two available email under the Sender email option on your notification settings.
Once you have changed your settings, please test an actual submission and check if that could fix the issue you are having.
If it does, your email server/provider is having the issue and your form is working fine.
