May we know the website URL where your forms are embedded so we can check it on IE browser? If you're embedding the form to Facebook page please make sure to use secure form (https) to avoid security errors.

Safari on mac is working perfect.
The pc I'm checking runs windows 7 and IE 10
I am able to see your form in a live Windows 7 emulator with IE10
The only thing that I've noticed is your current DOCTYPE declaration, which is
<!DOCTYPE HTML PUBLIC "-//W3C//DTD HTML 4.01 Frameset//EN" "http://www.w3.org/TR/html4/frameset.dtd">
Can you please updated it to
<!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">
