I created the following form using JotForm: http://www.cameronrosecollection.com/Contact_Us.html
The e-mail address that those submissions are sent to is email@example.com, but when I tried to reset my password with that address, I get an error message that that e-mail is not in your system. Can you help?

Have you created a free account before? I have checked your email address and only found a guest form related to your email address.
To create a free account, you will see something like this on the left side when you go to My Forms page:
Click the orange Create a Free Account button and go through the process of creating your account.
You can also find the long orange button just below the login/password fields:
Note: Make sure to use the email address that you have used in your form so that the form you have created will be merged to your new free account.

You seem to have created this as a guest user. If you create an account with your email address, this form will automatically be assigned to your new account.
