I uploaded my contract (jpeg) and then added the e-sign option at the bottom and submitted the form. When I look at the submission I only see the signature and the contract itself if not there. Any idea why?

Upon checking your form "KY Alarm", I see that you have only two questions in your form. One is the e-signature and the other one is a image file you have added.
Please be informed that free text (HTML) and images of your form cannot be part of the submission data. Adding image question/field will only add an image in your form for display purpose. Only user input fields can be recorded in submission data.
You can achieve your requirement by adding your image in a radio button or checkbox. Here is a sample form I created for you. Have a look and and let us know if this is what you are trying to achieve: http://form.jotformpro.com/form/32938457783975?
