I have had several people let me know they have submitted forms and have not been e-mailed to me. This is the secong go around with this. I was able to view them on my on your site. Please let me know if I need to change someting to have my forms sent to email@example.com.

Hello, upon checking your account I noticed on your other form "Adoption Application Form", that you have 2 notification templates both are properly set with no problems. However, both have the same exact settings and message. It might be because of this one conflicting to each other. My advise is to delete one either one of them.
I have also cleared your form's caches from our end. I have also checked your only recipient email address and can not find it to our bounce list. This should work fine without problems.
