why is some information from my form missing in the notification email?

    two fields in my form (name and email address) are not displaying in the notification emails. please help.

    Hello tarasteger,

    When you edit your form's notification, it becomes dirty. That means after editing the notification alert, any fields/question you add in your form will not be added automatically in your notification and needs to be added manually.
    "Child's Name" and "Your Email Address" is not displaying any value in your submission emails because when you changed these fields in your form, the field names got changed but as you have edited your form's notification alert, it still has the old field names.
    The most convenient way to solve this problem is to delete your notification and add a new one. When you add a new notification alert, you will see that all question of your form will match with the notification fields.
