Can you tell me which method you used to edit those form submissions? Did you edit them in the submissions page (pictured below)
or using an Edit Link such as this - https://www.jotform.com/form.php?formID=3492826713&sid=17267051897868846&mode=edit
I tried replicating your issue by editing form submissions but it fails to create a duplicate entry.
Could it be possible that the user submitted twice? I see duplicate entries with slightly different data. For example, the entry for Lexis Belding.
I'll be awaiting your response. Let me know how this goes.

If my memory serves me correctly, this issue is usually caused by having a duplicate or duplicates of the form in question in the trash. You actually have quite a number of forms in the trash. Restore the ones you wish to keep and permanently delete the ones you don't. If you're sure you don't want to keep any of the forms, just click "Empty Trash".
Please let us know if this solves the problem and if we may be of additional assistance.
EDIT: Now that I see Neil's response, I believe I may be getting this confused with another issue.
