Okay, I just need to signup on the webpage and I assume there is something in the form to ensure HIPPA is complied with

You need to sign up for an account and build a form with desired questions. If you want to build HIPAA-compliant forms, you need to upgrade your account to HIPAA compliance. Please note that HIPAA compliance is only available in silver and Gold plan. You need to first upgrade your account and then upgrade to HIPAA compliance. The following guides should help you:
