My form is not populating all the fields after the build. Not sure why. Heres the build then look at the link for the results

    The issue is caused because of the conditions you have added in form. You have added condition to show / hide fields, but the conditions seems to have error. Please check the screenshot below:

    I would suggest you to please delete both these conditions from your form and that should solve your problem.
