Workflow: Why does this include only the answers?

I added the workflow and integrated it, when I receive the workflow email, I am only seeing the answers, not the questions. below is what I receive.

I have been performing some tests and currently this includes only the answer to the question; however, note that it's possible to edit the content to manually add the label:
