Hi I notissed that the form is not functioning properly when accessed for the first time or with clear cache/cookies.
the payment field is not calculating the total
the upload attachement field is not working
and the calendar is not appearing
if I refresh the page everything works fine, but then when I clear my brawsers cache and go to my form same problem happens again.
Is this normal? should I mention it on my form that the page should be refreshed in order to correctly fill the form?

I tried refreshing the page but I am unable to reproduce the issue you are having.
I have cleared your forms caches. I would suggest to re-embed your form using iframe embed code. This should help fixed this issue. Getting the Form iFrame Code.
If problem persists upon re-embeding, please let us know so that we could further investigate.

so I tryed iframe but when I use iframe the form appears incomplete (my website actually display half of the form)
So I re-embedded the form (not in iframe) but I still have the same problem
to reproduce the problem please do the following:
clear your browsers cache and cookies (I go to firefox options and clear cookies and browsing history) then go to my website on the page with the form with this link http://greece.pas.org/DaysOfPercussion2014/Registrations
and I see that
the payment field does not calculate the total
the calendar dont appear
the upload attachment buttons dont upload
so then I simply refresh the page and everything is working just fine
the problem is that the people who are going to visit my website to submit this form will not refresh the page, they will simply see a form that is not working
for example if I go to my website on the page with the form from a browser that I don't use (for example Opera of Internet Explorer) the first time that I will see the form it will be not functional, if I refresh the page though it may work, sometimes I even need to refresh the page several times
so, I'm not talking about my form's cache, but about my browser's cache
please do this and let me know if you see the same problem

If you are having part of the form cut off using the iFrame Method, do try update the height value as of the iframe embed code so that the form is displayed fully.
This is how the code shows the whole form at this time I

tha code that you provide is not correct, I had to change the URL in the code in order to have the form dispayed, so now the form is dispayed correctly and I don't have the issue
but I noticed another problem, when I fill the form and upload the two documents I see that the form somehow becomes longer and the submit button actually disappears !
so I had to add some extra empty fields below the submit button, to give the form some extra space for after it is filled for the submit button to be still visible
is there another way to solve this problem without having to add those extra empty fields?
ok, I deleted the extra fields but left the hight size on the code, so now the form is dispayed bigger, with the extra space, so the submit button is visible when the documents are uploaded

That's great to hear that you were able to get your embed code to work. With regards to your second issue about adjusting the spaces in you form may we request that you open a new thread so that we can better track and maintain one topic per each thread and also to avoid confusion for those who reads the ticket.
